JH finished typing out Kathy’s and JH’s Family Tree, which was then sent (shortly afterwards) in the form of a letter to Kathy: see Family Tree letter on Electronic file  Action: JH to copy in Sara Stacey electronically.
 



2005 - October 19th - Wednesday

JH received copy of ‘Identification Letter’ from Kathy; Kathy has already sent this letter to Maggie in her Dialysis Unit; Maggie’s surname is Farrell.

JH called into the PALS (Patient Advisory and Liaison Service) Office and spoke to Pat Banks, Bereavement Administrator, who works a half-day on Wednesdays in the office.  JH asked as to whom JH should name as the definitive person to whom she should give her consent for disclosure of Medical Records; PB stated that, although this disclosure was not because JH was entering into litigation, it would still be someone in the Legal Department;  PB looked in the Internal Telephone Book and came up with Jill Whiting – whom JH declared she knew well!  Action: JH to contact Jill Whiting.  JH also spoke to Liz Herman, PALS Officer – admittedly Liz was in a hurry to collect her 16-year-old son at 1705hr, but JH was rather alarmed at Liz’s look of dismay when the former told her of JH’s plan to donate a kidney!  JH asked Richard to look up, on the Hospital’s Address Database, the postal address of Southend General Hospital which he did finally find under Southend Hospital NHS Trust.
